Chartwells and MSD of Warren Township will work with parents and caregivers to provide reasonable, individualized accommodations for students with allergies, dietary intolerances, or other dietary restrictions. For students who require dietary accommodations, please have your medical provider fill out a Special Diet Statement.  If the student has a food allergy requiring medication in the event of a reaction, have your medical provider fill out both a Special Diet Statement and a Food Allergy & Anaphylaxis Emergency Care Plan form.

Q. Will my student’s dietary needs be accommodated?

A. Chartwells and MSD of Warren Township are committed to making reasonable dietary modifications and or food substitutions for students with documented food allergies and or a medical condition that may require such modifications. However, in order to modify meals, schools must have a written medical statement, the Special Diet Statement form, on file signed by a licensed medical provider.

Q. How do I let schools know my child has dietary needs?

A. Provide a completed Special Diet Statement to your school’s nurse. This form covers food allergies, food intolerances, and/ or other dietary needs. The form must be filled out by a licensed medical provider.

Q. Medication is required if my student has an allergic reaction to a food, should I let the school know?

A. Yes! Have your medical provider fill out a Food Allergy & Anaphylaxis Emergency Care Plan (FARE) specifying the medication and dosage, in addition to the Special Diet Statement. Students with food allergies requiring medication require both forms.

Q. My student has a dairy sensitivity and is able to have some dairy products; just not drinkable cow’s milk. Is this possible with school meals?

A. Yes. On the Special Diet Statement, please have your medical provider specify if it is only drinkable cow’s milk or if it’s all food items with dairy ingredients. All students will be offered soy milk as an alternative to cow’s milk.

Q. My student can’t have cow’s milk or soy milk, is there another alternative?

A. Yes. Per State guidelines, almond milk may be served as an alternative to soy or cow’s milk IF requested by a medical provider and in case of a soy allergy.

Q. My student has a dietary restriction due to religious or cultural beliefs, will accommodations be made for my student?

A.If modifications can be made to the meal that comply with the USDA meal patterns and nutrition standard, Chartwells will modify meals to meet dietary restrictions due to religious or cultural beliefs. To request modifications please submit a Religious/ Cultural Diet Restriction form to your school’s nurse.

If your student no longer needs any meal modifications please contact your school’s nurse and complete and return a Discontinuation of Special Dietary Accommodations form. If your student’s dietary restrictions have changed, please have your medical provider complete an updated Special Diet Statement and return to the school’s nurse.

Accommodations will not be made for students requesting food modifications for personal food preferences including general health concerns and lifestyle choices. Please see the Non-Disability Related Accommodations Requests sheet for more information.
